The Zoom link for Concluding Service and Havdalah and the Shofar Blowing are the same. If you have already signed up for Concluding Service you do not need to sign up again for Havdalah. 

Simply stay on after Concluding and we will go straight into Havdalah and the Shofar Blowing. 

Yom Kippur being a "Sabbath of Sabbaths" ends with Havdalah, the ceremony that marks the distinction between holy and mundane time, and with the blowing of the shofar to usher in a new and sweet year.
